Guiding the Lower Jaw What Is a Herbst Appliance?

The Herbst appliance helps correct an overbite caused by a lower jaw that sits too far back. It attaches to the upper and lower back teeth and gently holds the lower jaw in a forward position while the child is still growing.

Because it's fixed in place, it works 24 hours a day, encouraging the lower jaw to grow into a better position.

Frequent Questions Learn More About the Herbst Appliance

A Herbst appliance corrects an overbite caused by a lower jaw that sits too far back. Small rods and tubes on each side hold the lower jaw in a forward position, which encourages it to grow forward and improves how the upper and lower teeth fit together.

Because it's fixed in place, it works all day and night without needing to be taken in and out.

It works best during a growth spurt, typically around ages 9 to 14, when the lower jaw is growing quickly and responds well to guidance.

It's attached with orthodontic bands or stainless steel crowns fitted around select teeth. The rod-and-tube mechanism connects to those bands or crowns. Some designs also include an expansion screw to widen the upper jaw.

Most children wear it for about 9 to 12 months. Afterward, braces or Invisalign finish aligning the teeth and fine-tune the bite.

Most kids adjust within about a week. Chewing feels different at first and the cheeks may be tender. Soft foods help during the first few days.

Brush carefully around the bands and rods after every meal, floss daily and use a fluoride rinse at night. Avoid sticky, hard and sugary foods. If anything feels loose, call our office.
